1st method:

Your Bank Address,

Dear Mr/Ms/Madam/Sir,

Body of your letter, in here you can state why were you unable to do so and the reason why you to activate your account.

2nd method:
Some banks offer their letter to there website. You might want to check it out first if it is available, this would make things a lot easier.

As far I know you can just go to your bank, then request your reactivation directly to the counter that handle this kind of action.
